It extends the ideas of Model Driven Development. Conceptual independence eases adaptability by a separation of the Business domain terms from the functions that use the terms in the application. Although in practice many examples can be found in which parts of this approach are applied, a consistent application of this idea is not often encountered. The principle of Conceptual independence is sketched in this paper in a pattern. Since developing information systems often depends on the content of the Business domain, and the conceptual model of the domain needs frequent adaptation, systems need to be adaptable. In this paper we describe a complete decoupling of the software system from its conceptual model of the domain. We illustrate the relevance of this approach for the professional practice of software development and architecture. Furthermore, we describe related approaches for adaptable systems.

2014, Article / Letter to editor (Journal of Nursing Scholarship, vol. 46, iss. 3, (2014), pp. 187-198)Purpose To describe the concurrent incidence of pressure ulcers, urinary tract infections, and falls in hospitals and nursing homes, and the preventive care given. Additionally, the correlation between the occurrence of these adverse events and preventive care was explored. Design and Settings A prospective, 3-month, cohort study at 10 hospitals and 10 nursing homes in the Netherlands. Participants 687 hospital patients and 241 nursing home patients. Main Outcome Measures The incidence of three adverse events and preventive care given to patients at risk. During weekly visits, the patients and their files were assessed. Additionally, observations were performed. Results Seventy-seven hospital patients (11%) and 111 nursing home patients (46%) developed one or more adverse events. The incidence rate for both types of patients, and for the three adverse events combined, was 9% adverse events per patient week. In hospitals, 34% of the patients received adequate pressure ulcer preventive care, while 47% of the patients received adequate urinary tract infection preventive care, and none of the patients received adequate falls preventive care. In nursing homes, 18% of the patients received adequate pressure ulcer preventive care, 42% of the patients received adequate urinary tract infection preventive care, and less than 1% of the patients received adequate falls prevention care. Negative or no correlations were found between the incidence rates for the three adverse events. In nursing homes the incidence of pressure ulcers and preventive care were positively correlated. Conclusions There is a high incidence of adverse events in hospitals and nursing homes. Many patients at risk do not receive adequate preventive care.

Consensus verkrijgen over het mobiliseren van patiënten op de intensive care (IC) qua definitie, hoofd- en specifieke doelstellingen, verrichtingen, frequentie, indicaties en contra-indicaties. METHODE. Een expertgroep van verpleegkundigen, fysiotherapeuten en artsen van IC-afdelingen van Nederlandse ziekenhuizen is samengesteld volgens de quotamethode. Een Delphi-onderzoek is uitgevoerd onder de expertgroep (n = 64). De mate van consensus over de aspecten van het mobiliseren werd vastgesteld in drie rondes (= 75%: overtuigende consensus, 60 tot 75%: consensus, < 60%: geen consensus). RESULTAAT. De respons op de drie ronden bedroeg respectievelijk 87,5% (n = 56), 87,5% (n = 57) en 76,5% (n = 49). (Overtuigende) consensus is bereikt over de definitie, de hoofddoelen, dertien van de negentien specifieke doelen, acht van de tien verrichtingen, de frequentie van het positioneren (minstens tweemaal per dag) en van het oefenen en transfers (tweemaal per dag). Overtuigende consensus is bereikt over de indicatie van het mobiliseren (iedere patiënt mits een contra-indicatie). (Overtuigende) consensus is bereikt over verhoogde intracraniale druk, hemodynamische instabiliteit en instabiele wervelfracturen als contra-indicaties voor het mobiliseren. (Overtuigende) consensus is bereikt voor respiratoire instabiliteit, saturatiegraad, FiO2, open thorax en verstoring van cerebrale functies (met vermelding van grenswaarden) als contra-indicaties voor transfers. CONCLUSIE. De resultaten van dit onderzoek vormen de eerste aanzet tot het ontwikkelen van een multiprofessionele richtlijn over het mobiliseren van beademde patiënten op de IC. Aanbevelingen zijn gedaan voor de revalidatie van patiënten na ontslag van de IC en uit het ziekenhuis.
2020, Article / Letter to editor (Arthritis Research & Therapy, vol. 22, iss. 1, (2020), pp. 54)BACKGROUND: The concept of osteoarthritis (OA) heterogeneity is evolving and gaining renewed interest. According to this concept, distinct subtypes of OA need to be defined that will likely require recognition in research design and different approaches to clinical management. Although seemingly plausible, a wide range of views exist on how best to operationalize this concept. The current project aimed to provide consensus-based definitions and recommendations that together create a framework for conducting and reporting OA phenotype research. METHODS: A panel of 25 members with expertise in OA phenotype research was composed. First, panel members participated in an online Delphi exercise to provide a number of basic definitions and statements relating to OA phenotypes and OA phenotype research. Second, panel members provided input on a set of recommendations for reporting on OA phenotype studies. RESULTS: Four Delphi rounds were required to achieve sufficient agreement on 11 definitions and statements. OA phenotypes were defined as subtypes of OA that share distinct underlying pathobiological and pain mechanisms and their structural and functional consequences. Reporting recommendations pertaining to the study characteristics, study population, data collection, statistical analysis, and appraisal of OA phenotype studies were provided. CONCLUSIONS: This study provides a number of consensus-based definitions and recommendations relating to OA phenotypes. The resulting framework is intended to facilitate research on OA phenotypes and increase combined efforts to develop effective OA phenotype classification. Success in this endeavor will hopefully translate into more effective, differentiated OA management that will benefit a multitude of OA patients.
2011, Book (monograph) In dit onderzoek staat de onderzoeksvraag centraal: is het mogelijk tot een consensus-based document te komen voor dramatherapie met angststoornissen? Dit onderzoek is een vervolg op een ontwikkelingsonderzoek uit 2005 waarin ik het fasenmodel van Emunah aanbeveel voor dramatherapie bij angststoornissen. Het ontwikkelingsonderzoek beperkte zich tot het werk van één dramatherapeut met een open groep met sociale angst, paniek- en dwangstoornissen. Dit consensus-based onderzoek is breder van opzet. Voor dit onderzoek zijn 8 dramatherapeuten geïnterviewd die met angststoornissen werken, waarbij alle angststoornissen aan bod komen, dus ook de posttraumatische stressstoornis (PTSS). Er zijn verschillende dataverzamelingstechnieken gebruikt om de ervaringskennis van deze professionals zo goed mogelijk te expliciteren en te beschrijven. Hierbij komen alle vormen van dramatherapie aan de orde: open en gesloten groepen en individuele therapie. Deze beschrijvingen zijn geabstraheerd tot behandelproducten die op te vatten zijn als best practices. Het voordeel van behandelproducten is dat je de concrete praktijk kan abstraheren tot algemeen geldende uitspraken. Een nadeel is dat het soms nog maar weinig zegt over die praktijk. Vandaar dat ik de behandelproducten demonstreer met levendige en waarheidsgetrouwe procesbeschrijvingen die tot de verbeelding spreken en tesamen een soort handboek vormen voor dramatherapie met angststoornissen. Dit handboek vormt de basis voor het consensus-based document en biedt tevens veel inspiratie voor dramatherapeuten. In dit onderzoek worden ook de behandelingsvisies beschreven van de verschillende klinieken die van invloed zijn op de dramatherapeutische aanpak. Allereerst natuurlijk de cognitieve gedragstherapie (CGT) die in de multidisciplinaire richtlijn wordt aanbevolen. Gedurende het onderzoek bleek dat de acceptatie- en commitment-therapie (ACT) steeds meer in opmars is bij de aanpak van angststoornissen. Daarnaast is de schemagerichte therapie (SGT) ook groeiende bij angststoornissen. Verschillende dramatherapeutische werkvormen komen aan bod die geïnspireerd zijn op één van deze behandelingsvisies. Na analyse van het bovenstaande materiaal was het mogelijk tot de beschrijving van het consensus-based document te komen. Hierin zijn aspecten van de behandelingsvisies terug te vinden als gevoelens leren verdragen en negatieve cognities bewerken tot positieve cognities. Verder staat in dramatherapie het doorbreken van het vermijdingsgedrag centraal waarbij gezonde kanten versterkt worden en cliënten hun gedragsrepertoire leren uitbreiden. De kerngroep van dramatherapeuten beschouwde de fasenindeling van Emunah als een bruikbare dramatherapeutische richtlijn in het werken met angststoornissen, waarbij alle fasen aan bod komen afhankelijk van de vorm van de therapie, de specifieke angststoornis en het proces waarin de cliënten zich bevinden. Dit consensus-based onderzoek biedt een goed uitgangspunt voor een verdere effectstudie en biedt daarmee een bijdrage tot het evidence-based werken.

2021, Article / Letter to editor (BMC Musculoskeletal Disorders, vol. 22, iss. 1, (2021), pp. 633)BACKGROUND: We recently developed a model of stratified exercise therapy, consisting of (i) a stratification algorithm allocating patients with knee osteoarthritis (OA) into one of the three subgroups ('high muscle strength subgroup' representing a post-traumatic phenotype, 'low muscle strength subgroup' representing an age-induced phenotype, and 'obesity subgroup' representing a metabolic phenotype) and (ii) subgroup-specific exercise therapy. In the present study, we aimed to test the construct validity of this algorithm. METHODS: Data from five studies (four exercise therapy trial cohorts and one cross-sectional cohort) were used to test the construct validity of our algorithm by 63 a priori formulated hypotheses regarding three research questions: (i) are the proportions of patients in each subgroup similar across cohorts? (15 hypotheses); (ii) are the characteristics of each of the subgroups in line with their proposed underlying phenotypes? (30 hypotheses); (iii) are the effects of usual exercise therapy in the 3 subgroups in line with the proposed effect sizes? (18 hypotheses). RESULTS: Baseline data from a total of 1211 patients with knee OA were analyzed for the first and second research question, and follow-up data from 584 patients who were part of an exercise therapy arm within a trial for the third research question. In total, the vast majority (73%) of the hypotheses were confirmed. Regarding our first research question, we found similar proportions in each of the three subgroups across cohorts, especially for three cohorts. Regarding our second research question, subgroup characteristics were almost completely in line with the proposed underlying phenotypes. Regarding our third research question, usual exercise therapy resulted in similar, medium to large effect sizes for knee pain and physical function for all three subgroups. CONCLUSION: We found mixed results regarding the construct validity of our stratification algorithm. On the one hand, it is a valid instrument to consistently allocate patients into subgroups that aligned our hypotheses. On the other hand, in contrast to our hypotheses, subgroups did not differ substantially in effects of usual exercise therapy. An ongoing trial will assess whether this algorithm accompanied by subgroup-specific exercise therapy improves clinical and economic outcomes.

Hoe voer je als sociaal werker een gesprek met iemand die er niet voor kiest om met jou te praten?
Dit boek beschrijft de dagelijkse praktijk vanuit het proces en het perspectief van de sociaal werker: omgevingsgericht, dichtbij de cliënt en diens netwerk, handelend vanuit het bevorderen van eigen kracht, zelfredzaamheid en wederkerigheid en gebruikmakend van gereedschappen die ingezet kunnen worden bij dwang en drang. De centrale taak is om te zorgen voor werkelijk contact met de cliënt.

Shop selling stagnates. But as fashion sales handled through e-channels is becoming a substantial part of the business, succesfully managing such a channel is a challange for almost all fashion retailers. This research maps the logistics structures of the diffrent fashion e-tailers, and obtains insight in the patterns how product are distributed and returned. The involved logistics structures differ strongly from the logistics structures towards fashion shops in the high street, especially for its return flows. We describe which parts of the logistics flows are most vulnerable, and which KPI’s as compared with regular supply chains. Delivery appears to be an important part of the total customers experience. Controlling, applying KPI’s and improving efficieny during the e-fulfilment phase is considered essentially by companies to become more competitive and financially succesful. We found that the number of KPI’s seems tob e related tot he width of the product range and the price level. Our research gives a clear view how fashion chains are coping with e-logistics and how this relates to existing theoretical models. Especially the control on goods returned, which has not been done much before in-depth. This is important to know, as fashion chains urgently ask for a better understanding of the question show to manage e-tailing and which KPI’s may suit to what conditions. The research offers several practical recommendations tot he industry on this aspect.
2018, Article / Letter to editor (vol. 13, iss. 3, (2018))In face-to-face social interactions, blind and visually impaired persons (VIPs) lack access to nonverbal cues like facial expressions, body posture, and gestures, which may lead to impaired interpersonal communication. In this study, a wearable sensory substitution device (SSD) consisting of a head mounted camera and a haptic belt was evaluated to determine whether vibrotactile cues around the waist could be used to convey facial expressions to users and whether such a device is desired by VIPs for use in daily living situations. Ten VIPs (mean age: 38.8, SD: 14.4) and 10 sighted persons (SPs) (mean age: 44.5, SD: 19.6) participated in the study, in which validated sets of pictures, silent videos, and videos with audio of facial expressions were presented to the participant. A control measurement was first performed to determine how accurately participants could identify facial expressions while relying on their functional senses. After a short training, participants were asked to determine facial expressions while wearing the emotion feedback system. VIPs using the device showed significant improvements in their ability to determine which facial expressions were shown. A significant increase in accuracy of 44.4% was found across all types of stimuli when comparing the scores of the control (mean±SEM: 35.0±2.5%) and supported (mean±SEM: 79.4±2.1%) phases. The greatest improvements achieved with the support of the SSD were found for silent stimuli (68.3% for pictures and 50.8% for silent videos). SPs also showed consistent, though not statistically significant, improvements while supported. Overall, our study shows that vibrotactile cues are well suited to convey facial expressions to VIPs in real-time. Participants became skilled with the device after a short training session. Further testing and development of the SSD is required to improve its accuracy and aesthetics for potential daily use.

2015, Article / Letter to editor (BMC Psychiatry, vol. 15, iss. 100, (2015))Background This study aims for health gain and cost reduction in the care for people with long-term non-psychotic psychiatric disorders. Present care for this population has a limited evidence base, is often open ended, little effective, and expensive. Recent epidemiological data shows that 43.5% of the Dutch are affected by mental illness during their life. About 80% of all patients receiving mental health services (MHS) have one or more non-psychotic disorders. Particularly for this group, long-term treatment and care is poorly developed. Care As Usual (CAU) currently is a form of low-structured treatment/care. Interpersonal Community Psychiatric Treatment (ICPT) is a structured treatment for people with long-term, non-psychotic disorders, developed together with patients, professionals, and experts. ICPT uses a number of evidence-based techniques and was positively evaluated in a controlled pilot study. Methods/Design Multi-centre cluster-randomized clinical trial: 36 professionals will be randomly allocated to either ICPT or CAU for an intervention period of 12 months, and a follow-up of 6 months. 180 Patients between 18–65 years of age will be included, who have been diagnosed with a non-psychotic psychiatric disorder (depressive, anxiety, personality or substance abuse disorder), have long-term (>2 years) or high care use (>1 outpatient contact per week or >2 crisis contacts per year or >1 inpatient admission per year), and who receive treatment in a specialized mental health care setting. The primary outcome variable is quality of life; secondary outcomes are costs, recovery, general mental health, therapeutic alliance, professional-perceived difficulty of patient, care needs and social contacts. Discussion No RCT, nor cost-effectiveness study, has been conducted on ICPT so far. The empirical base for current CAU is weak, if not absent. This study will fill this void, and generate data needed to improve daily mental health care.
2015, Article / Letter to editor (International Journal of Nursing Studies, vol. 52, iss. 1, (2015), pp. 112-120)Background: No-rinse disposable wash gloves are increasingly implemented in health care to replace traditional soap and water bed baths without proper evaluation of (cost) effectiveness. Objectives: To compare bed baths for effects on skin integrity and resistance against bathing and costs. Design: Cluster randomized trial. Setting: Fifty six nursing home wards in the Netherlands. Participants: Five hundred adult care-dependent residents and 275 nurses from nursing home wards. Methods: The experimental condition 'washing without water' consists of a bed bath with disposable wash gloves made of non-woven waffled fibers, saturated with a no-rinse, quickly vaporizing skin cleaning and caring lotion. The control condition is a traditional bed bath using soap, water, washcloths and towels. Both conditions were continued for 6 weeks. Outcome measures were prevalence of skin damage distinguished in two levels of severity: any skin abnormality/lesion and significant skin lesions. Additional outcomes: resistance during bed baths, costs. Results: Any skin abnormalities/lesions over time decreased slightly in the experimental group, and increased slightly in the control group, resulting in 72.7% vs 77.6% of residents having any skin abnormalities/lesions after 6 weeks, respectively (p= 0.04). There were no differences in significant skin lesions or resistance after 6 weeks. Mean costs for bed baths during 6 weeks per resident were estimated at 218.30 (95%Cl 150.52-286.08) in the experimental group and 232.20 (95%Cl: 203.80-260.60) in the control group (difference 13.90 (95%Cl: 25.61-53.42). Conclusion: Washing without water mildly protects from skin abnormalities/lesions, costs for preparing and performing bed baths do not differ from costs for traditional bed bathing. 2020, Article / Letter to editor (BMJ Open Sport & Exercise Medicine, vol. 6, iss. 1, (2020), pp. e000943)Post-COVID-19 patients, particularly those who needed high care, are expected to have high needs for physical, psychological and cognitive rehabilitation. Yet, the resources needed to provide rehabilitation treatment are expected to be inadequate because healthcare systems faced a shortage of high-quality treatment of these symptoms already before the COVID-19 crisis emerged in patients with comparable needs. In this viewpoint, we discuss the potential of Virtual Reality (VR) administering fast, tailor-made rehabilitation at a distance, and offering a solution for the impending surge of demand for rehabilitation after COVID-19. VR consists of a head-mounted display (HMD) that can bring the user by computer-generated visuals into an immersive, realistic multi-sensory environment. Several studies on VR show its potential for rehabilitation and suggest VR to be beneficial in post-COVID-19. The immersion of VR may increase therapy adherence and may distract the patient from experienced fatigue and anxiety. Barriers still have to be overcome to easily implement VR in healthcare. We argue that embedding VR in virtual care platforms would assist in overcoming these barriers and would stimulate the spread of VR therapy, both for post-COVID-19 patients in the present and possibly for other patients with similar rehabilitation needs in the future.
2010, Article / Letter to editor (Advances in Science and Technology, 5th Forum on new materials Part C, iss. 74, (2010), pp. 297-302)In an previous research project a new type of greenhouse with an integrated concentrated photovoltaic system (CPV) was developed which has an integrated filter for reflecting the near infrared radiation (NIR) to the greenhouse and exploiting this radiation in a solar energy system. The performance of the system was promising. In this study further optimalisations of the CPV system are made to avoid the large construction for solar tracting. Hereto all parts will be integrated into the greenhouse. The NIR-reflector material is carried out as a NIR-reflective lamllea system and the CPV–module is mounted into the ridge. In this paper the results of the optimization process of the CPV system based on NIR reflecting lamellae is presented. The optimization process is based on a maximal total annual electricity production and is performed with a ray tracing model and actual radiation data. Results show that the optimization of the lamellae greenhouse can be seen from a theoretical and a practical point of view. Theoretically, the number of lamellae for the investigated concept must be high (>100) and focus with a generic focal length of 3.5 m and glazing bars must be avoided. Then the maximal annual electricity output can be over 26 kWh/m². In practice, mechanical restrictions, plant conditions and costs will determine the implementation. The proposed CPV-system has positive side-effects like reducing the heat load (and need for cooling) during summer and blocking of the direct radiation which can be harmful for some crops. With this, the feasibility of the system depends greatly on local conditions which require a tailor-made economical analysis
2010, Article / Letter to editor (AIP Conference Proceedings, iss. 1277, (2010), pp. 264-267)A new CPV system with a static linear Fresnel lens, silicon PV module suitable for concentrated radiation and an innovative tracking system is integrated in a greenhouse covering. The basic idea of this horticultural application is to develop a greenhouse for pot plants (typical shadow plants) which don’t like high direct radiation. Removing all direct radiation will block up to 77% of the solar energy, which will reduce the necessary cooling capacity. The solar energy focused on the Thermal Photovoltaic (PV/T) module generates electric and thermal energy. The PV/T module is tracked in the focal line and requires cooling due to the high heat load of the concentrated radiation (concentration factor of 50 times). All parts are integrated in a greenhouse with a size of about 36?m2. The electrical and thermal yield is determined for Dutch climate circumstances. Some measurements were performed with a PMMA linear Fresnel lens between double glass. Further improvement of the performance of the CPV-system is possible by using a PDMS lens directly laminated on glass and using AR-coated glass. This lens is developed with ZEMAX and the results of the Ray-tracing simulations are presented with the lens structure oriented in an upwards and downwards position. The best performance of the static linear Fresnel lens is achieved with upwards orientation of the lens structures. In practice this is only possible with the Fresnel lens placed between a double glass structure, which will keep the lens clean and free of water.

In dit project zijn een verpleegkamer, een operatiekamer en een couveusekamer onderzocht op materiaalgebruik. Dit derde artikel in de reeks C2C beschrijft het onderzoek “De Couveusekamer”. De doelstelling van het onderzoek is aanbevelingen te doen voor Cradle to Cradle materiaalgebruik met betrekking tot de gezondheid van de gebruikers van de couveuse.

In een latere uitgave van FMT zal het onderzoek naar een couveuse worden gepubliceerd. In het onderzoek “De Operatiekamer” van het Lectoraat Zorggericht Bouwen aan de Hogeschool van Arnhem en Nijmegen, is aandacht besteed aan het huidig gebruik van materialen in een operatiekamer en de mogelijke gevolgen hiervan op de mens en zijn omgeving met oog op het Cradle to Cradle principe. De centrale vraag daarbij was: hoe is het gesteld met duurzaamheid van toegepaste materialen in een operatiekamer? En welke Cradle to Cradle oplossingen zijn mogelijk?

Cradle to Cradle sluit hier op aan: producten moeten zodanig ontwikkeld worden dat deze na gebruik als grondstof kunnen dienen voor een nieuw product in de kringloop. De vraag is nu hoe wij bij het ontwerpen van de gebouwde leefomgeving in de gezondheidszorg gebruik kunnen maken van deze gedachte.

Toch is ondernemerschap voor veel creatieven nog steeds vooral een noodzakelijk kwaad om creatieve doelen te bereiken. Het gevolg is een weinig professionele invulling van het ondernemerschap die het creatief succes bedreigt. In hoeverre wijkt creatief ondernemerschap af van ‘gewoon’ ondernemerschap? Creatieven definiëren zich zelden als ondernemers omdat ze hun culturele toegevoegde waarde doorgaans belangrijker vinden dan de economische. Pleidooi voor aandacht voor de samenhang en wisselwerking tussen ‘culturele winst’ en ‘economische waarde’.

Veel steden proberen zich in die strijd aantrekkelijk te maken als creatieve stad. Ze doen er alles aan om kunstenaars en designers aan te trekken, knappen oude fabrieken op en ontwikkelen kekke wijken met hippe winkels. En dan? Ben je dan een creatieve stad? Onzin, zegt ARCCI-lid Roy van Dalm. Een creatieve stad is meer dan een creatieven stad. Het is pas het begin. Maar steden die het niet gesnapt hebben, houden het hier bij en beginnen te klagen dat het een hype is. Gelukkig weten wij in Arnhem wel beter, toch? Waar de Creatieve Stad w‘rkelijk om draait, laat dit ARCCI seminar zien.

Purpose
This mixed-method study aims to contribute to the scholarly debate by outlining an individual-level theoretical framework for public value creation and evaluation that builds upon a social exchange perspective. It provides insights into the normative frames of primary stakeholders in the Dutch care at home sector, that is professionals, managers, clients and informal care providers.
Design/methodology/approach
A mixed-method design comprising a customized survey among 349 stakeholders, preceded by 31 in-depth interviews was used.
Findings
This empirical work shows differences and similarities in the stakeholders' normative frames revealed via three dimensions of expectations regarding the process of care delivery: personal contact, impact of rules and procedures and communication.
Social implications
These differences in interpretation have implications for the measurement and evaluation of public value creation.
Originality/value
By statistically and methodologically exploring the different expectations' scales that are developed, we intend to work toward a measure for public value creation, which can be used in future empirical work.
