Gait analysis before and after corrective osteotomy in patients with knee osteoarthritis and a valgus deformity.

2017, Article / Letter to editor (Knee Surgery, Sports Traumatology, Arthroscopy, vol. 25, iss. 9, (2017), pp. 2904-2913)PURPOSE: In this prospective study, the changes in kinetics and kinematics of gait and clinical outcomes after a varus osteotomy (tibial, femoral or double osteotomy) in patients with osteoarthritis (OA) of the knee and a valgus leg alignment were analysed and compared to healthy subjects. METHODS: Twelve patients and ten healthy controls were included. Both kinetics and kinematics of gait and clinical and radiographic outcomes were evaluated. RESULTS: The knee adduction moment increased significantly postoperatively (p < 0.05) and almost similar to the control group. Patients showed less knee and hip flexion/extension motion and moment during gait pre- and postoperatively compared to the controls. A significant improvement was found in WOMAC [80.8 (SD 16.1), p = 0.000], KOS [74.9 (SD 14.7), p = 0.018], OKS [21.2 (SD 7.5), p = 0.000] and VAS-pain [32.9 (SD 20.9), p = 0.003] in all patients irrespective of the osteotomy technique used. The radiographic measurements showed a mean hip knee ankle (HKA) angle correction of 10.4° (95 % CI 6.4°-14.4°). CONCLUSION: In patients with knee OA combined with a valgus leg alignment, the varus-producing osteotomy is a successful treatment. Postoperatively, the patients showed kinetics and kinematics of gait similar as that of a healthy control group. A significant increase in the knee adduction moment during stance phase was found, which was related to the degree of correction. The HKA angle towards zero degrees caused a medial shift in the dynamic knee loading. The medial shift will optimally restore cartilage loading forces and knee ligament balance and reduces progression of OA or the risk of OA. A significant improvement in all clinical outcomes was also found. LEVEL OF EVIDENCE: III.

The researchers use two generic patterns based on the identification of an object and based on the relation between two objects to examine the FCO-IM efficacy. Study revealed that the data modelling problems found from the structures of universe of discourse (UoD) collected fact can be solved using FCO-IM.
Georganiseerde begeleiding bij opleiden in de school : Een onderzoek naar het begeleiden van werkplekleren van leraren in opleiding

2014, Dissertation Bij het opleiden van nieuwe leraren wordt steeds meer belang toegekend aan het leren in de beroepscontext. Met het Opleiden in de School (OidS) zijn grote stappen gezet om in samenwerkingsverbanden tussen scholen en lerarenopleidingen werkplekleren als onderdeel van de opleiding te organiseren. Het blijkt een veelbelovend, maar eveneens complex onderdeel van de opleiding. Om de kwaliteit hiervan te borgen wordt onder meer ingezet op het organiseren van begeleiding van de leraar in opleiding. In de afgelopen jaren hebben scholen en lerarenopleidingen afspraken gemaakt over het organiseren van begeleiding. Begeleiding kan echter op veel manieren worden uitgewerkt en ingezet, waarbij het niet duidelijk is welke strategie leidt tot welke opbrengst (Hobson et al., 2009). Het onderzoek in dit proefschrift heeft als doel meer zicht te krijgen op de praktijk van de begeleiding van het werkplekleren. Centraal staat de vraag hoe het georganiseerde begeleiden vorm krijgt bij het OidS in de context van de initiële opleiding van leraren voor het voortgezet onderwijs.

Gyroscopic management as added value for management

2012, Part of book or chapter of book (, pp. 61-86)In recent publications in International Journals in 2010 and 2011, I described the phenomenon of a new approach to education and training called ‘Gyroscopic Management’ (See list publications). To give the reader an insight and overview of this, I will introduce and explain this gyroscopic management and, based on literature research and my own experiences, I describe how a currently used approach in education and the training of business and management students at Arnhem Business School (ABS) shows that this can lead to added strategic value for management in organizations. This also represents the start of a so-called, ‘Grounded Action’ research about the influence of training and educating for new future managers in organisations, as described in the second part of this paper. The research took place in Romania and The Netherlands and it shows the discovered five stages of ‘gyroscopic management’: “Learning, listening, awareness, accepting, adapting, and advancing.” I explored this explanatory theory further into a so-called ‘Grounded Action’ research approach in which I made an ‘operational theory’ (Simmons & Gregory 2003). This research is implemented in the form of a final applied research in educational and training practice during trainings, lectures at ABS in the Netherlands and at the West University in Timisoara, Romania (UWT).

Het is de door mannen meest beoefende zaalsport en de meest beoefende sport door vrouwen (in georganiseerd verband). In Nederland wordt handbal door zo’n 60.000 mensen gespeeld. 70 Procent hiervan is vrouw. Om de jeugd enthousiast te maken voor handbal is het essentieel dat ze al op jonge leeftijd in aanraking komen met de sport. Onderwijs is daarvoor de meest geschikte ingang.

In artikel drie besteden we aandacht aan schijnbewegingen, verdedigen en aan tactische principes. De lesstof in dit artikel is meer geschikt voor de vaardigere handbalspelers. We adviseren het toepassen van schijnbewegingen, verdedigen en tactiek aan bod te laten komen in het derde schooljaar van het voortgezet onderwijs of als leerlingen worden voorbereid op een schoolhandbaltoernooi.

New ways of thinking about nursing leadership and evidencebased human resource management are required to improve nursing environments. Conclusion: Health-promoting leadership may be a promising path to optimizing nursing outcomes through holistic thinking, which emphasizes the importance of context. 2010, Article / Letter to editor (TVZ : verpleegkunde in praktijk en wetenschap, vol. 2010, (2010), pp. 42-46)Op de pijnpoli van de Alysis Zorggroep te Arnhem vraagt de arts tijdens het consult aan de patient naar diens medicijngebruik. Dit kost tijd en patienten weten hun medicatie vaak niet volledig op te sommen of goed te beschrijven. Het afleveroverzicht van de openbare apotheek geeft slechts de afgeleverde recepten weer. Om de overdracht van medicatiegegevens te verbeteren werd een pilotonderzoek uitgevoerd met als vraagstelling of een Actueel Medicatieoverzicht (AMO) dat door de patient thuis wordt ingevuld een juister overzicht iedt van het daadwerkelijk medicijngebruik dan de huidige werkwijze. Het gerandomiseerde onderzoek met controlegroep werd in de periode mei tot en met juli 2008 uitgevoerd op de pijnpoli van de Alysis Zorggroep te Arnhem.

In this framework the Dutch government has recently compiled a policy document, ambitiously stating that the Netherlands should become the most creative economy in Europe by 2020. I must say that I am not always happy with this typical habit of policy makers nowadays to translate every ambition in sports terms. However, if it must be done, it is crucial to use the right yardstick. So I posed the question: “which economy is the most creative now, and what can we learn from it?”

2016, Article / Letter to editor (Disability and Rehabilitation, vol. 38, iss. 3, (2016), pp. 211-217)PURPOSE: To explore rehabilitation professionals' opinions about the influence and the pathways of injury compensation (IC) on health and disability in patients with whiplash associated disorder (WAD). METHODS: Semi-structured interviews were performed among a purposeful selected sample of Dutch expert-professionals in the field of rehabilitation of patients with WAD. Inclusion continued until saturation was reached. Inductive and deductive thematic analyses were performed. RESULTS: Ten rehabilitation expert-professionals (five females), working as physician, psychologist or physiotherapist, were interviewed. All expert-professionals acknowledged that IC can influence rehabilitation, health and disability. The expert-professionals provided three causal pathways; a pathway through prolonged distress, a behavioral pathway, and patient characteristics that may either attenuate or worsen their response. They assess the influence of IC mainly with interview techniques. Most professionals discuss the potential influence of IC with their patients, because they want to give clear information to the patient. Some emphasize that their role is neutral in relation with the IC. Others mention that financial consequences can accompany functional improvement. CONCLUSIONS: Rehabilitation expert-professionals believe that IC may affect rehabilitation, health and disability in patients with WAD. Three pathways are mentioned by the experts-professionals. IMPLICATIONS FOR REHABILITATION: According to rehabilitation expert-professionals, an injury compensation (IC) can lead to distress, by creating a (conscious or unconscious) conflict of interests within a patient between striving for compensation on one hand, and recovery on the other hand. Patient characteristics can either attenuate or worsen IC-related distress. Reliable and valid tools need to be developed to assess the influence of IC on health, disability and rehabilitation, and to limit the negative effects. Rehabilitation professionals can discuss the possible unintended effects of IC with their patients to clarify their current situation.

A qualitative study with purposive sampling and semi-structured interviews. Methods. Data were obtained by interviewing 10 Dutch nursing experts in the field of cognitive function in older patients. The interviews were recorded, transcribed and analysed by two independent researchers. All the respondents stated that daily observation of cognitive function yields valuable information. The concept of cognitive function was operationalised differently by institute and by nurse. Observation and reporting methods varied, as did the goals set by the nurses. Nurses reported using many days of observation to reach final judgements. Observations of cognitive functioning should include several cognitive domains, be restricted to a few days of observation and aim to both contribute to medical diagnoses and guide nursing interventions.

Identities of research-active academics in new universities : towards a complete academic profession cross-cutting different worlds of practice

2017, Article / Letter to editor ((2017), pp. 1-17)This study explores how academics who expanded their teaching-only positions to include research view their (re)constructed academic identity. Participants worked in a higher professional education institution of applied research and teaching, comparable with so-called new universities. The aim is to increase our understanding of variations in academic identity and to be better able to support academics’ ‘role making’ within and across different worlds of practice. Data from semi-structured interviews with 18 academics at a Dutch new university were analysed using a grounded theory approach. This revealed six well-rounded academic identities reflecting participants’ personal scholarly objectives: the ‘continuous learner’, ‘disciplinary expert’, ‘skilled researcher’, ‘evidence-based teacher’, ‘guardian of the research work process’ and ‘liaison officer’. The researcher role served to promote the overall development of participants’ identities. The ‘disciplinary expert’ matured through participation in the academic world and research activities. Participants discovered what ‘being’ and ‘becoming’ a researcher in the new university might entail, and contributed to the professions’ knowledge base. Participants learned to apply various research-based teaching approaches. As brokers, they linked research projects to practices in meaningful ways. The six identities embodied an emergent power in creating and preserving a complete academic profession. Participants’ accounts showed tensions inherent in an extended role portfolio and constraints in ‘role making’ given inconsistencies between the university’s espoused research mission and the one in use. These imply challenges for university managers in aligning policies and practices, and scaffolding academics’ attempts to integrate their academic roles in different worlds of practice.

Research and clinical evidence in cancer have shown that nutritional support can reduce length of hospitalisation, diminish treatment-related toxicity, and improve nutrient intake, quality of life, and physical function. Nutritional intervention can improve outcomes and help patients in the successful completion of oncological treatments by preventing malnutrition. Malnutrition is a very common hallmark in patients with cancers. Almost one-fourth of cancer patients are at risk of dying because of the consequences of malnutrition, rather than cancer itself. Patients with digestive cancers are at higher risk of suffering malnutrition due to the gastrointestinal impairment caused by their disease. They are at high nutritional risk by definition, yet the majority of them have insufficient or null access to nutritional intervention.Inadequate resources are dedicated to implementing nutritional services in Europe. Universal access to nutritional support for digestive cancer patients is not a reality in many European countries. To change this situation, health systems should invest in qualified staff to reinforce or create nutritional teams' experts in digestive cancer treatments. We aim to share the patient community's perspective on the status and the importance of nutritional intervention. This is an advocacy manuscript presenting data on the topic and analysing the current situations and the challenges for nutrition in digestive cancers. It highlights the importance of integrative nutrition in the treatment of digestive cancers and advocates for equitable and universal access to nutritional intervention for all patients.

Without the use of a delirium screening instrument, delirium is often missed by ICU nurses and physicians. The effects of implementation of a screening method on haloperidol use is not known. The purpose of this study was to evaluate the implementation of the confusion assessment method-ICU (CAM-ICU) and the effect of its use on frequency and duration of haloperidol use.

The sprinting and dribbling performances, measured by time scoring, (n=14) were tested before and after a twelve week intensive training period. Performance changes were analyzed by means of t-tests for paired data. A correlation matrix described the relation between the changes from T1 to T2 on sprinting and dribbling. Sprinting and dribbling improvements are not directly related to each other, future training programs should contain more specificity and should also be individualized to increase performance improvements.
